A RESOLUTION TO APPROVE THE SPECIAL NEEDS ASSISTANCE PROGRAM GRANT
AGREEMENT AMENDMENT WITH THE U.S. DEPARTMENT OF HOUSING AND URBAN
DEVELOPMENT AND FAYETTEVILLE HOUSING AUTHORITY IN THE TOTAL AMOUNT
OF $76,763.00 FOR THE CITY TO ADMINISTER PROGRAMS TO ASSIST HOMELESS
FAYETTEVILLE RESIDENTS
WHEREAS, the grant agreement amendment will transfer Project Number AR0071L6F0I 19001 to
the City of Fayetteville and keep this homeless assistance program viable.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
FAYETTEVILLE, ARKANSAS:
Section 1: That the City Council of the City of Fayetteville, Arkansas hereby authorizes Mayor Jordan
to sign Special Needs Assistance Program Grant Agreement Amendment with the U.S. Department of
Housing and Urban Development and Fayetteville Housing Authority in the total amount of
$76,763.00 for the City to administer programs to assist homeless Fayetteville residents.

SUBJECT: Special Needs Assistance Program (SNAPS) Grant Agreement Amendment
RECOMMENDATION:
Staff recommends approval of the Grant Agreement Amendment Project Number
AR0071 L6F0119001 and approval for the Mayor to execute the grant agreement amendment.
BACKGROUND:
The grant agreement amendment will transfer Project Number AR0071 L6F01 19001 to the City
and keep this homeless assistance program viable. The NWA CoC approached the City of
Fayetteville to ascertain the program capacity to implement the grant and prevent the loss of
grant funds that support the vulnerable population of those experience homelessness.
DISCUSSION:
The grant agreement amendment Project Number AR0071 L6F011900 in the amount of $76,763
will transfer the project to the City of Fayetteville.

AMENDMENT TO THE CONTINUUM OF CARE GRANT AGREEMENT
This Grant Agreement is made by and between the United States Department of Housing
and Urban Development (HUD) and Fa etteville Housing Authority, I N School Ave
Fayetteville, AR 72701, the Recipient, whose Tax ID number is 71-0398535 and DUNS Number
809755059 and the Replacement Recipient City of Fayetteville, 113 West Mountain
Fayetteville, AR 72701, whose Tax ID number is 716018462 and DUNS Number 141310578
RECITALS
1. HUD and the Recipient entered into the Grant Agreement dated August 11, 2021, for
Project Number:
AR0071L6F011900 to be located at Fayetteville, Arkansas, and Northwest Arkansas CoC
Area
2. The Recipient no longer will continue to be the Recipient of the Grant Agreement
because Recipient is being replaced.
3. The Replacement Recipient has submitted evidence acceptable to HUD that the
Replacement Recipient is eligible to be a recipient of a Continuum of Care program grant
and meets the capacity criteria in the Notice of Funding Availability under which the
grant was awarded.
4. The Replacement Recipient has submitted to HUD all required Application documents
and certifications; and all required Technical Submission documents, including
certifications, assurances, information and documentation required to meet any
conditions, which HUD has approved.
5. HUD has determined the Replacement Recipient should assume the obligations of
Recipient for the remainder of the term of the Grant Agreement.
6. The parties are desirous of amending the Grant Agreement to change the recipient.
AGREEMENTS
The Grant Agreement is hereby amended as follows:
1. The Recipient is hereby removed as recipient and replaced with the Replacement
Recipient.
2. The definition of the term "Application" is amended to include all certifications and
documents submitted by the Replacement Recipient to HUD, on the basis of which HUD
approved replacing the Recipient with the Replacement Recipient.
3. Notices to the Replacement Recipient shall be directed to Yolanda Fields, Community
Resource Director, City of Fayetteville, 113 West Mountain, Fayetteville, AR 72701
4. HUD's total funding obligation for the AR0071L6F011900 Permanent Supportive
Housing grant this project is $76,563 and current balance S76763 is allocated as follows:
1. Grant for operating $
2. Grant for supportive services $20,000
3. Grant for leasing $
4. Grant for rental assistance $53,352
5. Grant for HMIS $500
6. Grant for administration $2,911
This Amendment to Grant Agreement constitutes the entire agreement of the parties as to
amendment of the Grant Agreement and will become effective only upon the execution hereof by
all parties. The remaining terms of the Grant Agreement remain in full force and effect.
