FOR IMPROVEMENTS TO A PUBLIC FACILITY BUILDING IN THE AMOUNT OF $3,300.00 AS PART OF THE COMMUNITY DEVELOPMENT BLOCK GRANT SUBRECIPIENT PROGRAM BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF FAYETTEVILLE, ARKANSAS: Section 1. That the City Council of the City of Fayetteville, Arkansas hereby approves a change order, a copy of which is attached as Exhibit "A," to a contract with Joe Perme Construction, Inc. for improvements to a public facility building in the amount of $3,300.00 as part of the Community Development Block Grant Subrecipient Program. Garland PROPOSAL: A contract with Joe Penne Construction for the moderate rehabilitation of 1932 S. Garland was approved on June I, 2010, in resolution #93-10. Once work on the project began, it was determined that a change order would be needed in order to complete the rehabilitation of the property due to the discovery of some unexpected problems. 1) The scope of work stated that the kitchen cabinets were to be removed with only a portion of them replaced. Once the cabinets were removed, it was discovered that the materials used on the wall behind the cabinet area was different than the walls in the rest of the kitchen area. To make the kitchen area uniform in appearance the wall behind the cabinet area needs to be removed and replaced with drywall. Some areas of drywall also need to be patched and skim coated. 2) The scope of work stated that the entry deck would be replaced. Once the entry deck was removed, it was discovered that the sill plate was rotten and not able to be used for the replacement of the deck. If left uncorrected the flooring will begin to sag under the door and cause structural problems in the area. To repair the problem, some siding must be removed and jacks installed to support the weight of the structure while a section of the sill and bottom plate are replaced and leveled. Jacks will need to be removed and siding returned to its original condition. RECOMMENDATION: Staff recommends approval of change order for Public Facility Project located 1932 S. GENERAL: The contractor will be responsible for all aspects of the construction, installation, repairs, and cleanup as stated in the Scope of Work and Specifics at the project location stated above. Each bidder will be responsible for a complete inspection of the property at the project location before submitting bid. All the distances, measurements, procedures, and listed materials should be verified before submitting your bid to complete the work. The requirements listed here, in the description of work, are general requirements and it will be the responsibility of the winning bidder to assure the City of Fayetteville that the finished project is in compliance with all applicable codes and standards and achieves the goal of raising the efficiency of the building. The contractor's duties and responsibilities include, but are not limited to, the following: 1) Purchasing of all materials. 2) Supervision of all employees and subcontractors. 3) Coordination with the City of Fayetteville CDBG Administrator, the City of Fayetteville Code Compliance Office and owners. 4) Project design. 5) Clean up and restoration of all lawns, planters, walking and driving surfaces to their original condition. 6) Keep job site safe for others and practice safe working practices as required. SPECIFICS: A: ELECTRICAL A - 1 Replace all interior light fixtures with white surface mount fixtures, except for fan in office 4. Lights in offices and living rooms should be 3 bulb fixtures. Replace light fixtures in bathrooms. All bulbs should be energy efficient compact florescent light bulbs (CLS's). Some lights may need different solutions to optimize lighting plan. A - 2 Add ground fault plugs in kitchen by the sink Per. code 210.50 B.3, 2008 National Electric Code A - 3 Add any missing covers on switches or plugs. A - 4 Remove kitchen stove plug and vent hood and cap off electricity for addition of cabinets. B: INTERIOR DOORS B - 1 Remove and replace the doors entering the following numbered offices. 1,9,10,11,12,13,14,16,17. B - 2 Remove and replace the doors entering the following numbered bathrooms. 2,3,4 B - 3 Replace or add closet doors in the following numbered offices. 9,10,11,13,17. B - 4 Remove and replace the two full size closet doors in hallway number 19. Do not replace the half doors. B - 5 Reinstall all old door knobs. C : KITCHEN REMODEL AND STORAGE CABINETS C - 1 Tear out all kitchen cabinets and storage cabinets or shelves in the following numbered offices: 1,4,6,7. C - 2 Tear out all ceiling soffits in kitchen. C - 3 Add 10 feet of new base cabinets in kitchen in area where sink is now. Install new upper cabinets over the base cabinets (when area is exluded because of the window the uppers are about a total of 6 ft. (there will be no stove area) C - 4 Install and plumb new sink and faucet under window. (Similar sink to what is in now) D: DRYWALL D - 1 Remove shinny white wall paneling in hallway #19. Repair possible existing drywall behind paneling or redrywall the walls if not existing. Drywall the ceiling. The whole hallway should have a uniform look. D - 2 Repair the drywall in areas that had the cabinets or shelves removed. E: INTERIOR TRIM AND PAINT E - 1 Trim all added doors. Add trim against any ceilings that were drywalled that have paneling on the walls. Add baseboards to any rooms that do not have them. E - 2 Paint all interior doors that have been added and paint old doors that have not been painted very recently. E - 3 Paint all newly added or repaired drywalled areas. E - 4 Paint the ceilings in the kitchen and in the following numbered offices. 4,10,12,13,14. Room number 10 needs the one ceiling tile tacked up straight before painting. Paint the ceiling in bathrooms 3 and 4. E - 5 Paint or replace the few bad looking ceiling duct vents. F: BATHROOM REPAIRS F - 1 Remove and replace all toilets with new ones. (In bathroom # 5 install full size toilet) F - 2 Replace all bath vanity cabinets and tops. In bathroom # 5 install full size pedestal sink. F - 3 Install and plumb new vanity faucets F - 4 Remove old mirrors and install new ones over sinks. G: FLOORING G - 1 Remove all existing carpet. Remove all existing hard flooring unless the new flooring can be installed over it in a smooth way. G - 2 . Install new commercial carpet in all offices, living rooms, and storage rooms. (Approximately 2700 square feet) G - 3 Install new commercial vinyl flooring or similar product in the kitchen, utility room, both hall ways, and all bathrooms. (Approximately 1092 square feet) H: ENTRY DECKS H - 1 Remove the two entry decks and steps on the east side and replace them with new treated lumber. I: CLEANUP I - 1 All new, used, and excess construction materials that relate to the job or a change order(s) belongs to the contractor and can be removed from the property by the contractor as needed during the project and shall be removed at completion. I - 2 All trash shall be hauled off by contractor and preferably recycled if possible. I - 3 All affected areas shall be left broom clean. I - 4 If lead paint is present, cleanup will be compliant with EPA/HUD Approved Lead Safety standards. J: OTHER J - 1 All work or materials which are not directly noted in the Scope of Work and Specifics, but are necessary for the proper carrying out of the obvious intentions thereof, are to be understood as implied work and will be provided for by the contractor as if specifically described or drawn. J - 2 All work shall be performed in a workmanlike manner according to common construction practices, according to the specifications set forth in the Community Services Program's General Specification Manual, and with adherence to city, state, and national codes. J - 3 Any damage to the dwelling or property caused by the contractor, his/her worker(s) or delivery person, and/or their vehicles during the project shall be repaired to like new conditions. J 4 Where applicable, all plumbing work shall be performed by a Arkansas State licensed plumbing contractor with adherence to the current Arkansas Plumbing Code. J 5 Where applicable, all electrical work shall be performed by a Arkansas State licensed electrical contractor with adherence to the current National Electrical Code. J - 6 Where applicable, all HVAC work shall be performed under the supervision of a licensed HVAC contractor with adherence to all codes.
