AMENDMENT TO THE ARKANSAS BLUE CROSS AND BLUE SHIELD
GROUP CONTRACT

Schedule A of the Arkansas Blue Cross and Blue Shield Group Contract is amended to show the rates
effective January 1, 1997
Charges for each enrolled employee shall be as follows:

Initial charges shall be paid on January 1, 1997 and no coverage shall be in effect until such payment

thereafter.
This Amendment becomes a part of the Arkansas Blue Cross and Blue Shield Group Contract.

A RESOLUTION APPROVING THE OPTION TO RENEW
BCBS GROUP PLAN FOR POLICY YEAR 1/1/97 THROUGH
12/31/97 AND THE ADDITIONAL PREMIUM COST.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
FAYETTEVILLE, ARKANSAS:
Section 1. That the City Council hereby approves exercising the City's option to
renew BCBS Group Plan for policy year 1/1/97 through 12/31/97 and approves the premium
increase of 17.67%.
