BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF FAYETTEVILLE, ARKANSAS: 5ection 1. That the Mayor and City Clerk are hereby authorized and directed to execute a change order with McClinton - Anchor in the amount of $150,300.00 for the construction of the Town Creek Garden on the West Avenue parkIng lot. A copy of the change order authorized for execution hereby ls attached hereto marked Exhibit "A" and made a part hereof. Section 2. CHANGE ORDER INSTRUCTIONS A. GENERAL INFORMATION This document was developed to provide a unifot m format for handling contract changes that affect Contract Price or Contract Time. Changes that have been initiated bv a Work Directive Change must be incorporated into a subsequent Change Order if they affect Price or Time. Changes that affect Contract Price or Contract Time should be promptly covered by a Change Order. The practice of accumulating change order items to reduce the adminis- trative burden may lead to unnecessary disputes. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Time, a Field Order may be used. B. COMPLETING THE CHANGE ORDER FORM Engineer initiates the form. including a description of the changes involved and attach- ments based upon documents and proposals submitted by Contractor, or requests from Ovvner. or both. Once Engineer has completed and signed the form, all copies should be sent to Contractor for approval. After approval by Contractor, all copies should be sent to Owner for approval. Engineer should make distribution of executed copies after approval by Owner. If a change only applies to price or to time, cross out the part of the tabulation that does not apply. ATTACHMENT NO. 1 DESCRIPTIONS: Item 1. The construction of Town Creek Gardens Including a pavilion, with a skylight, for public use, in accordance with detailed plans attached. The work includes all work and materials necessary to improve: landscaping; irrigation system; earthwork; stone retaining walls and reconstruct the 'Arch'; spring box modifications to pump spring water to the ponds; electrical work; sidewalks and ramps; flatwork and all necessary utility connections to the pavilion. The total cost for this change Is $146,000.00 Item 2. Replace existing three rail aluminum handrail for the retaining wall with a seven rail aluminum handrail. The price for this item is $4,300.00 . PURPOSE OF CHANGE ORDER: Item 1. To incorporate the spring, landscaping, irrigation and pavilion Into the West Avenue site. Item 2. It also increases in size, and changes the variety of six trees in the Town Creek Garden. This change order also extends the contract time to May 15, 1992. Description of Change Order: Item 1. Additional concrete paving required at Spring Street near tie-in for truck turning for warehouse. The total increase in cost for this item is $280.0O. Item 2. Item 3. Delete solid sod requirement between sidewalk and parking lot along Dickson Street and a portion of West Avenue from the site of the new pavilion and Dickson Street. The total decrease in cost for this item is $375.00 . Add sidewalk at westerly side of entrance to existing concrete parking lot of Dickson Street. The total increase in costs for this item is $425.00 . Item 4. Add handicap ramp north of southeast entrance off West Avenue. The total increase for this item is $200.00 Item 5. Delete upper portion of traffic control anchor. The total decrease in cost for this item is $450.00. Item 6. Delete the Skylight Frame System and laminated safety glazing - 16'9" X 11'4" (Kawneer 1600 S.G. Framing system with *40 dark bronze anodized finish). The total decrease in costs for this item is $11,219.00 . Page 1 • • Purpose of Change Order: Item 1. • The area involved in this item appeared to be in a much more deteriorated state once debris was removed and construction commenced. The addition of paved area thus allows full use of truck parking and turning area as designed. Item 2. This area will be planted and maintained by a private organization and therefore will not require solid sod. Item 3. This sidewalk is being added to allow pedestrian traffic traverse to the west, along Dickson Street, on a paved surface to the University area. Item 4. This item was added to comply ADA requirements by providing direct access to the east across West Avenue Item 5. The City of Fayetteville traffic department. after considering the concerns of Walton Arts Center and adjacent properties, decided that a better location for the pole and anchor would be in the westerly sidewalk of West Avenue. Therefore, construction of the pole anchor was halted and credit was given for the remainder of the work. Item 6. The style and configuration of the skylight was changed to reduce the cost of this item. Also changes were made in the roof structure to simplify construction and reduce costs. Attachments: Item 6.
