FOR THE CONSTRUCTION OF THE ARTS CENTER PARKING LOTS ON SCHOOL AVENUE AND DICKSON STREET. BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF FAYETTEVILLE, ARKANSAS: Section 1. That the Board of Directors hereby approves Change Order No. 1 to the contract with McGoodwing, Williams and Yates, Inc. for the construction of the Arts Center parking lots on School Avenue and Dickson Street. GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Time. Changes that have been initiated by a Work Directive Change must be incorporated into a subsequent Change Order if they affect Price or Time. Changes that affect Contract Price or Contract Time should be promptly covered by a Change Order. The practice of accumulating change order items to reduce the adminis- trative burden may lead to unnecessary disputes. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Time, a Field Order may be used. B. COMPLETING THE CHANGE ORDER FORM Engineer initiates the form, including a description of the changes involved and attach- ments based upon documents and proposals submitted by Contractor. or requests from Owner, or both. Once Engineer has completed and signed the form. all copies should be sent to Contractor for approval. After approval by Contractor, all copies should he sent to Owner for approval. Engineer should make distribution of executed copies after approval by Owner. If a change only applies to price or to time. cross out the part of the tabulation that does not apply. ATTACHMENT NO. 1 Descriptions: Item 1. Replace existing asphalt pavement section as shown on sheet 9 of the Plans with 1 1/2" of Asphalt Concrete Hot Mix Surface Course on 4 1/2" of Asphalt Concrete Hot Mix Binder Course. Contractor shall proof roll entire site, undercutting, replacing and testing subgrade as required to ensure stability for pavement placement. The original contract provided for payment at the rate of $6.50 per truck yard for all undercut operations in excess of 100 cubic yards. This is hereby changed to provide for a unit price item of 4,650 truck yards for undercut operations at the revised price of $5.60 per truck yard, for a total estimated amount of $26,040. Item 2. Increase the hydraulic capacity of the storm drainage system at the Dickson Street site to intercept storm flow and divert it away from under existing buildings on Dickson Street. See revised Structure and Pipe Schedules, 'Exhibit A.' The total increase for this change is $41,037. Item 3. Replace existing driveway section with sidewalk and integral curb at School Avenue site. The total increase for this change is $2,450. Purpose of Change Order: Item 1. To aid in expediting the paving portion of this project, increase pavement strength and to ensure completion of the project on schedule Item 2. Increase the hydraulic capacity of the storm drainage system at the Dickson Street site to intercept storm flow and divert it away from under existing buildings on Dickson Street. Item 3. Replace approximately 800 square feet of existing driveway section with sidewalk and integral curb at School Avenue site. Attachments: Item 2.
