RESOLUTION NO. 124-91
A RESOLUTION AUTHORIZING CHANGE ORDER NO. 11
FOR THE WALTON ARTS CENTER PROJECT.
BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF
FAYETTEVILLE, ARKANSAS:
Section 1. That the Mayor and City Clerk are hereby
authorized and directed to execute Change Order No. 11 in the
amount of $356,582.00 for the Walton Arts Center Project to provide
construction and/or installation of the sound system, window
treatment, stage lighting control, generator, fire alarm,
structural items, and chair lift site. A copy of the change order
authorized for execution hereby is attached hereto marked Exhibit
"A" and made a part hereof.

Attached please find twenty (20) originals of Change Order No. 11 for the Walton
Arts Center Project. This change orders reinstate the following items:

Fire Alarm, Structural items, Chair Lift
Site Work.
Private Fund Account
$356,582.00
These were deleted originally to allow the contract to be awarded and are being
added at this time to help complete the project in a timely manner.
The Arts Center Council has approved adding these change orders at this time.
These will require the City's approval along with the University of Arkansas'
approval. Please return the signed copies as soon as possible.

CHANGE ORDER NO. 11
Gentlemen:
You are hereby authorized to make the following changes in the Work
included in the referenced. Agreement with the City of Fayetteville and
the University1of Arkansas:
1. Refer to Paragraph 7.3.2, Item e. Reinstate this item of Work
which consists of the Sound Reinforcement System (Addendum No.
4, Item No. 5).
2. Refer to Paragraph 7.3.2, Item 1. Reinstate this item of Work
which consists of the Window Treatment (Addendum No. 4, Item
No. 16).
3. Refer to Paragraph 7.3.2, Item u. Reinstate a portion of this
item of Work consisting of the Stage Lighting Control System,
except for the preparation of shop drawings and installation of
special boxes which were reinstated in Change Order No. 8.
With this action all items of Work included in Addendum No. 4,
Item No. 28, have been reinstated in the contract.
4. Refer to Paragraph 7.3.2, Item v. Reinstate this item of Work
which consists of the Emergency Electrical Generator (Addendum
No. 4, Item No. 29).
5. Refer to Paragraph 7.3.2, Item w. Reinstate this item of Work
which consists of Fire Alarm Equipment (Addendum No. 4, Item
No. 30).
6. Increase the thickness of the concrete street approach from
West Avenue to the parking lot from 6" to 7" thick.

7. Add reinforcement and anchoring along north side of building
adjacent to the balcony lobby and at the second floor line and
in accordance with the attached drawing.
8. At the cantile lered angle between Grid JJ and Grid KK add a 4"
x 4" x 1/4" steel tube hanger from W18 x 35 above located 12"
from the end aof the angle as noted on Jacksonville Steel
Company structural steel shop drawing 185.
9. Install a chat

a. The chairlift shall be equal to Cheney Handi—Lift
Vertical Wheelchair Lift with a floor—to—floor height
of approximately 29-1/2". Chairlift shall include all
standard features including emergency stop, alarm,
manual loweringdevice, overspeed brake, key switch
operation at upper and lower level as well as in the
car, and a lift capacity of 750 pounds. It shall also
include electro—mechanical locked gates at top and
bottom landings.
b. The chairlift shall accept 120 volt, single phase,
electrical service for 1/2 HP motor.
10. Perform a portion of the sitework which had been omitted from
the contract by provisions of Addendum No. 5. This work shall
include the excavation necessary to bring the site to the rough
grade called for in Addendum No. 5. The quantity of work as is
determined from the ground elevation survey prepared by
McClelland Consulting Engineers on May 1, 1991, and includes
the excavation and removal of approximately 1,885 cubic yards
of material.
11. Install a conduit system for future computer wiring and power
as called for in Addendum. No. 7.
Accordingly, the contract will be

Drawings and Specifications.

The following Addendum shall be incorporated in the Drawings and Speci-
fications which are a part of the Contract Documents and shall super-
sede anything contained therein with which it may conflict.
Item No. 1. Refer to Drawing MEP 1.1, Utility Site Plan
a. Electrical Contractor shall provide a 4" conduit with pullwire
for computer network system from southwest portion of building exiting
from south wall of Electrical Equipment Storage 126 over to northwest
exterior wall of existing office building, 18" minimum below grade.
Terminate conduit 6'-0" above finished grade and cap. Verify exact
termination point with Owner. Contractor shall determine optimum
routing of conduit.
b. Electrical Contractor shall provide a 4" conduit with pullwire
from northwest exterior wall of existing office building over to
existing power pole to be relocated by SWEPCO near Spring Street and
new drive, 18" minimum below grade. Terminate conduit 6'-0" above
finished grade and cap at exterior wall of existing office building.
Verify exact termination point with Owner. Terminate and cap conduit
8'-0" above finished grade at relocated power pole. Verify location of
power pole with SWEPCO. Contractor shall determine optimum routing of
conduit.•
c. Electrical Contractor shall provide a 4" conduit with pullwire
from southwest portion of building exiting from south wall of
Electrical Equipment Storage 126 over to exterior wall of existing
education building, 18" minimum below grade. Terminate conduit 6'-0"
above finished grade and cap. Verify exact termination point with
Owner. Contractor shall determine optimum routing of conduit.
d. Electrical Contractor shall provide a 4" conduit with pullwire
from southeast portion of building exiting from southeast wall of
Mechanical 114 next to T.P.C. AHU-8 over to exterior wall of existing
F.O.C. Building (across School Avenue), 18" minimum below grade.
Terminate conduit 6'-0" above finished grade and cap. Verify exact
termination point with Owner. Contractor shall cut and patch pavement,
as required for installation of conduit under School Avenue.
Contractor shall determine optimum routing of conduit.
