A RESOLUTION AUTHORIZING THE MAYOR AND CITY
CLERK TO EXECUTE A CONTRACT WITH JORGENSEN AND
ASSOCIATES FOR ENGINEERING SERVICES TO
RECONSTRUCT ASH, EMMA,AND, LAVERNA STREETS.
BE IT RESOLVED BY THE BOARD OF DIRECTORS OF TBE CITY OF
FAYETTEVILLE , ARKANSAS:
Section 1. That the Mayor and City Clerk are hereby
authorized and directed to execute a Contract with Jorgensen and
Associates for engineering services to reconstruct Ash, Emma and
Laverna Streets. A copy of the Contract authorized for execution
hereby is attached hereto marked Exhibit "A" and made a part
hereof.
PASSED AND APPROVED this 20th day of June , 1989.

CONTRACT FOR !ENGINEERING SERVICES
IMPROVEMENTS TO
ASH, EMMA & LAVERNE STREET
FAYETTEVILLE, ARKANSAS
This arpement made, entered into and executed this the
CH7 day of ; 1989, by and between
Jorgensen & Associat hereinafter called "Engineer," and
the City of Fayetteville hereinafter called "Owner."
WITNESSETH:
Whereas, the Owner contemplates the improvements to Ash,
Emma & Laverne Streets in Fayetteville ,Arkansas.
Therefore the Owner and the Engineer in consideration of
the mutual covenants and agreements herein contained, do
mutually agree as follows:
SECTION 1 - EMPLOYMENT OF ENGINEER

The Owner agrees to empley the Engineer, and the Engineer agrees
to perform professional engineering services in connection with
the proposed improvements stated in the sections to follow
and for having rendered such services!, the Owner agrees to pay
the Engineer compensation as stated in the sections to follow.
SECTION II CHARACTER,AND EXTENT OF SERVICES
The Engineer Will perform the engineering services listed below:
1) the Engineer,shall perform all engineering and surveying work
required to produce construetion plans, specifications,
contract documents, cost estimate and other documents
necessary to Advertise for bids for improvements to Ash
street from Woolsey to Gregg street which is approximately
550'.

2) The Engineer shall supervise subgrade investigation on
Emma and Laverne Streets. This subgrade investigation
shall also include the interpretation of test results.
Subgrade testing shall include pavement thickness, base
thickness, cone penatrometer test on subgrade, soil
classifications. Soil testing shall be performed to
• determine the condition of base and subgrade. Test re-
sults and the interpretation of these results shall be
in the form of a report to the City.
3) The Engineer shall resist with bid advertisement and re-
commend a Contractor for Ash Street Improvements, conduct
• preconstruction conference, install survey stakes as
required monitor construction and provide "As -built" plans.
SECTION III - PAYMENT
For the work described under Section II - Character & Extent of
Services, the Engineer shall recieve payment as follows:
1) For the preparation of plans, specifications, contract
documents and the various items listed in Section II, item
No. 1, the Engineering and Survey fees are $ 1,600.00. This
amount is a not to exceed amount. This does not include the
bid phase. In addition, we will prepare plats for easements
and/or Right of Way Acquisition for $ 30.00/hr. and this
maximum fee will be $2000. Fees for preparation of plans,
specifications, contract documents and any plat preparation
shall be paid within 30 days of completion of this work.
2) For supervision of subgrade investigation on Emma & Laverne
Streets and interpretation of results, the Engineers fee will
be $ 1,020.00. This is a not to exceed amount. Fees for this
portion will be paid within 30 days of competion of this study.
3) For the bid phase and construction phase, the Engineers fee
will be $ 1,075.00. This is a not to exceed amount. Fees
for this portion shall be paid within 30 days of completion
of the project.
For payment purposes, the foliowing rates shall be used
Engineer - $ 35.00
Survey crew - $ 35.00
Technician/draftsman- $ 17.00
Secretary/typist - $ 12.00
Note that the above amounts do not include Health Department
review fees and material testing (soils compaction and
concrete testing). These fees will be paid for by the Owner
separate from the Engineering fees stated herein.
SECTION IV - CONTROL OF SERVICES
No work shall be done under this Contract until the Engineer is
Instructed in writing to proceed with the work.
SECTION V - SUCCESSORS AND ASSIGNS
The Owner and the Engineer each bind himself and his successors
in respect to all covenants of this Agreement in testimony of
which this agreement has been executed on behalf of the above
named Engineer and the above named Owner on the day and year
indicated.
