A RESOLUTION AUTHORIZING THE MAYOR AND CITY
CLERK TO EXECUTE A CHANGE ORDER IN THE CITY'S
SEWER CONSTRUCTION CONTRACT WITH FAYETTE TREE
AND TRENCH TO ADD REPLACEMENT OF THE EXISTING
SEWER MAIN ON OAK DRIVE.
BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF
FAYETTEVILLE, ARKANSAS:
Section 1. That the Mayor and City Clerk are hereby
authorized and directed to execute a change order in the City's
construction contract with Fayette Tree and Trench to add
replacement of the existing sewer main on Oak Drive. A copy of
the change order authorized for execution hereby is attached
hereto marked Exhibit "A" and made a part hereof.

Contractor: Fayette Tree and Trench, Inc.
The following changes are made to the Contract Documents:
450 L.F. OF 6" Sewer Line, 4 manholes, 240 s.y. street repair, and re-
connection of 10 service lines, to replace the existing sewer line on Oak
Drive.
Justification: The T.V. inspection of this sewer line has shown excessive
deterioration of the line. Existing services experience continual stoppage
problems and health hazards resulting from sewer' overflows. The County
Health Department has been monitoring this sewer line and informed us that
the problem needs immediate attention.
CHANGE TO CONTRACT PRICE•
Original Contract Price: $ 64,238.32
Current Contract Price, as adjusted by previous change orders: $ 64,238.32
The Contract Price due to this Change Order will be increase by$ 27,030.00
The new Contract Price due to this Change Order will be: $ 91,268.32
CHANGE TO CONTRACT TIME:
The Contract Time will be increased by thirty calendar days.
The Date for Completion of all work under the Contract will be 30 days from
the date of the approval of this change order.
APPROVALS REQUIRED:
To be effective, this order must be approved by the Owner if it changes the
scope or objective of the project, or as may otherwise be required under the
terms of the Supplement
