A RESOLUTION AUTHORIZING THE MAYOR AND CITY
CLERK TO EXECUTE CHANGE ORDER NO. 19 IN
CONTRACT "A" WITH OLSON CONSTRUCTION COMPANY
FOR IMPROVEMENTS TO THE CITY'S WASTEWATER
TREATMENT PLANT RESULTING IN A NET PRICE
INCREASE OF $4,757.00.
BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF
FAYETPEVILLE, ARKANSAS:
That the Mayor and City Clerk are hereby authorized and
directed to execute Change Order No. 19 to Contract A with Olson
Construction Company for improvements to the City's Wastewater
Treatment Plant resulting in a net price increase of $4,757.00. A
copy of the change order authorized for execution hereby is
attached hereto marked Exhibit "A" and made a part hereof.

The following modifications to the contract are hereby ordered:
Delete the barbed wire fencing and the three double gates shown on Drawing Sheet
S-1 at the Sludge Transfer Facility (CMIR #B-23). (The fence and gate at
entrance to site is unaffected.) ($2,098)
Change the air valves at laterals E-2 and S-1 on Drawing Sheet P 120 and at
laterals E-6 and S-4 on Drawing Sheet CD -P-210 to Type I, and change the blowoff
at laterals E-6 and S-4 on Drawing Sheet CD -P-80 to Type III per CMIR #8-27. $3,354*
Upgrade binges for the acoustical door in the Operations Building per CR #99 and
CMIR #B-28.
Change the reinforcing in the shotcrete-lined portions of the North and South
Interceptor Channels from No. 4 reinforcing bar to No. 6, 6" x 6" welded wire
mesh (CMIR #B-30). The reinforcing mesh shall conform to ASTM A 185, and tie
wire shall be 16 -gauge, black, soft -annealed wire.

*$1,462 apportioned for sludge laterals (grant eligible) and $1,892 apportioned for
effluent laterals.
This contract modification constitutes full and mutual accord and satisfaction for all
time and all costs related to this change. Hy acceptance of this contract modification,
the contractor hereby agrees that the modification represents an equitable adjustment to
the contract, and further, agrees to waive all right to file any further claims or changes
arising out of or as a result of this change, or the accumulation of executed Contract
Modifications on this Contract.
