BEND ITION NU. 30-87
A RESOLUTION AUTHORIZING THE MAYOR AND CITY CLERK TO
EXECUTE CHANGE ORDER B-3 IN THE CITY'S CONTRACT WITH
OLSON CONSTRUCTION COMPANY FOR THE SLUDGE MANAGEMENT
PROJECT AT THE WASTEWATER TREATMENT PLANT.
BE TT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF FAYEITEVILIE,
ARKANSAS:
That the Mayor and City Clerk are hereby authorized and directed to
execute Change Order B-3 in the City's contract with Olson Construction
Company for the Sludge Management Project at the Wastewater Treatment Plant.
A copy of the contract authorized for execution hereby is attached hereto
marked Exhibit "A" and made a part hereof.

The following modifications to the contract are hereby ordered:
Delete improvements to Wyman Road culvert crossing east'of Wyman Bridge shown (55,180)
in Detail 49 on Sheet P-408 of Contract Drawings and reroute outfall of'South
Interceptor Channel per CMIR #B-17 _.
Add electrical disconnects to transmission line to sludge site per CMIR 4B-18 5904
Add a single fence aate installation near monitoring Well No.3 per CMIR 48-19 5418
Revise North Interceptor Channel Chute per CMIR #B-22 (56,544)
Add thrust restraints to lateral risers per CMIR #B-25 55,552*
Extend the contract duration/completion dates 28 calendar days for adverse
weather throueh February 28, 1987, per CH2M HILL letter to Olson Construction
Comoany dated April 3, 1987.
TOTAL ($4,850)
*$1,818 apportioned for sludge laterals (grant eligible) and 53,734 apportioned for
effluent laterals.
This contract modifications constitutes full and mutual accord and satisfaction for all
time and all costs related to this change. By'acceptance of this contract modification,
the contractor hereby agrees that the modification represents an equitable adjustment to
the contract, and further, agrees to waive all right to file any Further claims or changes
arising out of or as a result of this change, or the accumulation of executed Contract
Modifications on this Contract.
