A RESOLUTION AUTHORIZING THE MAYOR AND CITY CLERK
TD EXECUTE CHANGE ORDER NO. 12 IN THE CITY'S CONTRACT
WITH OLSON CONSTRUCTION COMPANY FOR CONSTRUCTION
OF A NEW WASTEWATER TREATMENT PLANT TO RESOLVE
SEVEN (7) CONTRACT CHANGES RESULTING IN A NET
CREDIT TO THE PROJECT OF $5,276.04.
BE IT RESOLVED BY THE BOARD OF DIRBLROFS OF TBE CITY OF FAYFTPEVILLE,
ARKANSAS:
That the Mayor and City Clerk are hereby authorized and directed
to execute Change Order No. 12 in the City's contract with Olson Con-
struction Company for construction of the new Wastewater Treatment
Plant to resolve seven (7) contract changes resulting in a net credit
to the project of $5,276.04. A copy of the change order authorized
for execution hereby is attached hereto marked Exhibit "A" and made
a part hereof.

Change Furan mortar to type S mortar and waterproof S. wall of Primary P.S.
Chance Furan mortar to regular mortar and add letters to the Entrance Sign
Add 4-24" Elbows to Secondary Clarifiers - CMIR 411
Add special mounting for lab receptacles - CMIR #15
Change negative format from 4" x 5" to 6 cm x 7 cm and deduct 5 sets pictures -
Phase I pro rata share -
Change inlet grating #7 to
Adjust valve supports cost

assumed, $435.96 is actual)

pictures - Phase III pro rata share - CMIR 417

This contract modificationsconstitutes full and mutual accord and satisfaction for all
time and all costs related to this change. By acceptance of this contract modification,
the contractor hereby agrees that the modification represents an equitable adjustment to
the contract, and further, agrees to waive all right to file any further claims or changes
arising out of or as a result of this change, or the accumulation of executed Contract
Modifications on this Contract.
