RESOLUTION NO. I7-86
A RESOLUTION AUTHORIZING THE MAYOR AND CITY C'f.FRK
TO EXECUTE CHANGE ORDER NO. 5 AND CHANGE ORDER
NO. 7 IN THE CITY'S WASTEWATER TREATMENT PLANT
CONSTRUCTION CONTRACT WITH OLSON CONSTRUCTION
COMPANY.
HE IT RESOLVED BY THE BOARD OF DIREXIORS OF THE QTY OF FAYETTEVILLE,
ARKANSAS:
That the Mayor and City Clerk are hereby authorized and directed
to execute Change Order No. 5 and Change Order No. 7 in the City's
Wastewater Treatment Plant construction contract with Olson Construction
Company. Copies of the change orders authorized for execution hereby
are attached hereto marked Exhibits "A" and "B" and made a part hereof.
PASSED AND APPROVED this 21st day of January , 1986.

- Deepen roof beam at Primary Influent Pump Station (CR#51) 901.00
- Reroute plumbing in ceiling of Administration Building (CR#62) 1,180.00
- Plumb cup sinks at fume hoods in Administration Building (CR#83) 273.00
- Move W. reservoir dike east & steepen interior slope (CMIR*8) (Credit) (18,789.00)
-Add an 8" valve to strainer (CMIR#10) 789.00
- Add two control stations at Sludge Reservior (CR#52)
- Change Manhole #7 to drop manhole (CR#73)

This contract modification constitutes full and mutual accord and satisfaction
for all time and all costs related to this change. By acceptance of this contract
modification, the contractor hereby agrees that the modification represents an equitable
adjustment to the contract, and further, agrees to waive all right to file any further claims
or changes arising out of or as a result of this change or the accumulation of ahmogas on this

The following modifications to the contract are hereby ordered: (Use additional pages if
required)
Provide temporary utility hookups (water, gas; electricity, sewer) to the Administration
building to permit the Owner to occupy and use the building, and provide and maintain a
temporary gravel walkway to permit the Owners personnel access, per CMIR #12, in return for a
90 -day extension of time on the Administration Building. Said time extension includes any time
delay that may be granted due to weather occurring prior to October 1, 1985.
Provide sump pumps and sumps for 3 scum pump stations per CMIR #4 $11,388 - Phase I
Provide 14 outdoor lights around the existing plant per CMIR #4 $31,680 - Phase II
This contract modification constitutes full and mutual accord and satisfaction
for all time and all costs related to this change. By acceptance of this contract
modification, the contractor hereby agrees that the modification represents an equitable
adjustment to the contract, and further, agrees to waive all right to file any further claims
or changes arising out of or as a result of this change, or the accumulation of executed
contract modifications on this Contract.
