RESOLUTION NO. 71-85
A RESOLUTION AUTHORIZING THE MAYOR AND CITY CLERK
TO EXECUTE CHANGE ORDER NO. 4 IN THE CITY'S WASTEWATER
TREATMENT PLANT CONTRACT WITH OLSEN CONSTRUCTION
COMPANY TO PROVIDE FOR INSTALLATION OF A PERMANENT
BELT PRESS.
BE IT RESOLVED BY THE BOARD OF DIRECIDRS OF THE CITY OF FAYETTEVILLE,
ARKANSAS:
That the Mayor and City Clerk are hereby authorized and directed
to execute Change Order No. 4 in the City's Wastewater Treatment Plant
Contract with Olsen Construction Company to provide for installation
of a permanent belt press at a cost of $286,200.00. A copy of the
Change Order authorized for execution hereby is attached hereto marked
Exhibit "A" and made a part hereof.

The following modifications to the contract are hereby ordered: (Use additional pages if
required)
Add a Jones Belt Filter Press with new conveyor belt in conformance with the plans and
specifications contained in the request for proposals dated April 1985 as modified by the
proposal from Olson Construction Company dated July 1, 1985.
This contract modification constitutes full and mutual accord and satisfaction
for all time and all costs related to this change. By acceptance of this contract
modification, the contractor hereby agrees that the modification represents an equitable
adjustment to the contract, and further, agrees to waive all right to file any further claims
or changes arising out of or as a result of this change, or the accumulation of changes on this
Contract.

TASK ORDER NO. 4, AMENDMENT 1
INTERIUM SLUDGE ENGINEERING SERVICES
This Amendment No. 1 amends Task Order No. 4 executed
between CH2M HILL Southeast, Inc. "ENGINEER", and the City
of Fayetteville, Arkansas (OWNER), on the 19th day of
February, 1985.
Article 1
The scope of work for Task No. 2 is hereby amended to
include engineering services for:
o Addition and design of two Owner -furnished polymer
feed systems and two Contractor -furnished polymer
feed systems
o Demolition of the westerly lime slurry tank, and
repair of the hole left in the floor
o Relocation of the existing polymer storage tank
and the redesign of the polymer feed system to the
vicinity of the new belt filter press installation
o Move the stairway on the Jones belt filter press
alternative
o Make necessary piping revisions
Article 2
As consideration for providing the services enumerated in
Article 1 as amended, the cost ceiling shall be increased to
$55,900.
