RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF FAYEITEVILLE, ARKANSAS: That the Board of Directors hereby adopts the safety program described in Exhibit "A" attached hereto and made a part hereof. Mayor • , 1985. • A SUGGESTED MANAGEMENT STATEMENT OF SAFETY POLICY Thegp'ersctlal safety and health of each employee of this munici- pality is ofprimary importance. The prevention of occupation- ally-induced'injuries is of such consequence that it will be given precedence over operating productivity whenever necessary. To the greatest degree possible,.managemefit will provide all physical and mechanical facilities required for personal safety and health in keeping with the highest standards. we will maintain a safety program conforming with the best prac- t ices cf organizations of this type. To be successful, such a program must embody the proper attitudes towards injury preven- t ion on the'part of both supervisors and employees. It also requires cooperation in all safety matters between each employee and his fellow workers. It takes their cooperative efforts to establish and preserve a successful safety record. Our safety program will include: o Providing mechanical and physical safeguards to the maximum extent possible; o Conducting a program of safety inspections to find and remove unsafe working conditions or practices and to comply fully with the!safety standards for every job; o Providing necessary personal protective equipment and instructions for its use and care; o Training all employees in good safety practices; o . Developing and enforcing safety rules; requiring that employees cooperate with these rules as a condition of employment; o Investigating promptly =.nd thoroughly, every accident to find the cause and to take corrective action to prevent its reoccurrence We recognize that the responsibilites for safety are shared: o The employer is responsible and accepts the responsibility for leadership of the safety program, for its effectiveness and improvement, and for providing the safeguards required to ensure safe conditions; o Supervisors are responsible for developing the proper atti- tudes towards safety in themselves and in those they super- vise; and for ensuring that all operations are performed with the utmost regard for the safety of all personnel involved, including themselves; o Employees are responsible for complete, genuine cooperation with all aspects of the safety program - including compliance with all -rules and regulations - and for continuously prac- ticing safety while performing their duties. 6.01 N ��� • • 6.02 FAYETTEVILLE, ARKANSAS P. 0. DRAWER F 72701 (5011 521.7700 STEPS NECESSARY FOR IMPLEMENTATION OF SAFETY PROGRAM 1. Policy statement of commitment concerning accident prevention adopted by Board of Directors. 2. Policy statement of commitment concerning accident prevention, signed by Mayor published on City letterhead and distributed to each employee with paycheck. 3. Safety Committee of 5 members responsible for the direct administration of accident prevention formed. 4. A safety coordinator, appointed by the Committee to prepare the agenda and provide necessary exhibits, reports and factual material as well as record minutes. 5. Specific responsibilities and activities of the Safety Committee shall include: A.1 The primary specific function of this Committee shall be to discuss mayor accident -producing conditions and circumstances, and recommend practical effective corrective action. B.' Review of, and action on reports of the Safety Coordi- nator and suggestions by employees. Review of, and action on any reports and recommendations ✓ eceived from the representative of the Municipal League Workers Compensation Trust. D. Review of, and action on accident investigation and inspection reports submitted by supervisors. E. Periodic check of all authorized safety procedures and their proper functioning. F.) Approval of proposed new construction and installation o f equipment, changes in procedures and processes from t he safety viewpoint. 5A. The supervisor is charged with the responsibility for the prevention of accidents. He must know the safe methods and procedures of doing the work under his supervision. He must also know the safety rules and set an example for the other employees by practicing them. The supervisor shall explain in detail to new employees in: his department the safety rules and practices that apply to his duties. He will then follow up on his instructions .to make certain they are understood and carried out. • • 6.03 PAGE TWO SAFETY PROGRAM Supervisors will be held responsible for general housekeeping in and around the work area of which they are in charge. They will also be responsible for completing periodic inspection reports. 6. Each supervisor should conduct a short safety department meeting each month with all employees to cover current problem areas, talk about losses and a safety subject. 6A. Employees shall abide by the adopted safety rules at all times. - Prescribed wearing apparel as determined by management shall beworn while on the job. ii Employees are expected to observe generalhousekeeping rules, and help keep their work area neat and clean. Employees must report all injuries to their supervisors immediately. Employees are expected to provide cooperation and overall support to the Safety Program. 6B. Department head will take action when safety practices and policies set forth are not covered. 7. Inspection tours of the various operations will be made once a month in accordance with schedule set up by Safety Committee by Inspection Team. 8 Inspection Team will be made up of a Safety Committee member and two employees. The purpose of the Team Inspection Tour is"to bring to light unsafe conditions or acts and will also keep the idea or attitudeof accident prevention in the minds of the employees as they see the tour being conducted or asked about a condition or an act that might be considered hazardous. The team will make a written report of their findings to be given to the Safety Committee for discussion and solution at :.the monthly meeting. JC:jf
