Dear Sirs :
Officer Ruston L . Cole was examined by me on January 30 , 1997 , for neurosurgical
evaluation of his lower back problem . He has developed increasing difficulty
from this since November , 1996 , and has been unable to work since the first of
December , however he originally underwent lumbar disc surgery for the back
when he was in the Marine Corps in 1975 . The primary difficulty are residuals
of the lumbar disc problem aggravated by hypertrophic arthritis and bone spur-
ring , and he is getting lumbar nerve root compression , primarily at the L5-Sl
level , but he has degenerative disc disease with arthritic changes at the 1.3-4 ,
L4-5 , and L5-S1 levels .
It is my opinion that Officer Cole is disabled from carrying out the duties re-
quired of him as a police officer and his medical problems are of a permanent
nature .
